Description: This survey explores the extent to which children with special health care needs (CSHCN) have medical homes, adequate health insurance, access to needed services, adequate care coordination, and that parents are satisfied with their child’s care. Other topics may include functional difficulties, transition services, and questions.

Population: Children aged zero to 17 years with special health care needs in the 50 states and the District of Columbia.

Periodicity: Data available for the following collection periods: October 2000-April 2002, April 2005-February 2007, January 2009- March 2011, 2016-2017.
